Why this rating?

A statin: an HMG-CoA reductase inhibitor prescribed to lower LDL cholesterol. It has no anabolic, androgenic or ergogenic action and does not raise the natural ceiling by any path. It appears in this archive because AAS use, particularly oral 17-alpha-alkylated compounds, routinely wrecks the lipid panel, and this is the drug that addresses it. Rated 1 (basically natty).

Overview

One of the most potent statins available and the standard pharmaceutical answer to the lipid damage that steroid cycles cause. Oral 17-alpha-alkylated compounds in particular can crater HDL and drive LDL up sharply within weeks, an effect no amount of fish oil meaningfully offsets. This is the intervention that actually moves those numbers, and it is prescription medicine rather than a supplement.

Correcting Cycle-Induced Dyslipidemia

The reason it belongs in this archive. Androgens: especially oral 17-alpha-alkylated steroids, suppress HDL and raise LDL, often dramatically and quickly. A statin directly addresses the LDL side of that damage.

Long-Term Cardiovascular Risk Management

For anyone on long-term TRT or repeated blast-cruise cycles, cumulative atherosclerotic risk is the realistic long-horizon threat, not the acute sides people worry about. The lipid damage is the under-rated cost of a cycle

Most attention goes to acne, hair and gyno. The change that actually predicts long-term outcomes is the lipid panel. Androgens, and oral 17-alpha-alkylated compounds especially, can suppress HDL and raise LDL sharply within weeks. That damage is silent, cumulative and invisible without bloodwork.

It fixes LDL, not HDL

A statin lowers LDL very effectively. It does not restore the HDL that oral steroids suppress. Nothing reliably does. The actual fix for crushed HDL is coming off the compound causing it. Do not read a normalised LDL as "lipids handled".

Dose to a number, not to a feeling

Statin dosing is titrated against an actual measured lipid panel at 4-week-or-longer intervals. This is the clearest example in the archive of a compound where bloodwork is not optional, without it you are dosing blind.

Two hard dose caps most people miss

Patients of Asian descent should start at 5mg daily because plasma exposure is roughly 2-fold higher, and anyone with CrCl below 30 mL/min is capped at 10mg. Proteinuria also becomes most frequent at 40mg or above.

Dosing is defined by intensity rather than by user experience: 5-10mg/day is moderate-intensity (30-49% LDL reduction) and 20-40mg/day is high-intensity (≥50% LDL reduction). Titrate at intervals of 4 weeks or more, guided by an actual lipid panel rather than guesswork. Two important dose caps: patients of Asian descent should be initiated at 5mg once daily because plasma exposure is approximately 2-fold higher, and in renal impairment with CrCl <30 mL/min the starting dose is 5mg with a 10mg maximum. Proteinuria was most frequent at doses of 40mg or greater. This is not a compound to self-prescribe blindly. It is dosed to a measured lipid target under medical supervision.

Half-Life

Peak plasma concentration is reached 3-5 hours post-dose, with bioavailability of roughly 20% and 88% protein binding (primarily albumin). Around 90% is excreted via the biliary/fecal route and only ~10% renally. The long half-life is why once-daily dosing works and why timing relative to a workout or meal is irrelevant.

How It Works

Rosuvastatin inhibits HMG-CoA reductase, the rate-limiting enzyme in hepatic cholesterol synthesis. Reduced intracellular cholesterol causes the liver to upregulate LDL receptors, which pulls circulating LDL cholesterol out of the blood. The result is a dose-dependent reduction in LDL-C and total cholesterol, and a corresponding reduction in atherosclerotic cardiovascular risk.
