Why this rating?

Modafinil is a wakefulness-promoting drug used for sleep disorders and off-label as a cognitive/focus enhancer. It does not build muscle, burn fat or alter hormones - its effect is on alertness and, indirectly, training quality via reduced fatigue. The main caveat is that it is a WADA-prohibited in-competition stimulant, so it is not 'natty' for a tested athlete on the doping-eligibility axis even though it does nothing to your physique - it is rated 1 (basically natty) because it does not move the muscular/physique ceiling by any path.

Overview

A wakefulness-promoting medication approved for excessive sleepiness from narcolepsy, obstructive sleep apnea and shift work sleep disorder. Described as a CNS stimulant / atypical dopamine reuptake inhibitor, it is widely used off-label as a cognitive enhancer. Approved in the US in 1998 and sold as Provigil among other brands.

Purpose & Use Cases

Excessive Daytime Sleepiness

FDA-approved to treat excessive sleepiness caused by narcolepsy and obstructive sleep apnea/hypopnea syndrome.

Shift Work Sleep Disorder

Approved for shift work sleep disorder; taken about 1 hour before the start of a work shift.

Off-Label Cognitive Enhancement

Used off-label as a cognitive enhancer by students and professionals seeking improved focus and sustained wakefulness.

Benefits

  • Strong, sustained wakefulness without the crash of classic stimulants
  • Improved focus and reduced fatigue (basis of off-label cognitive use)
  • FDA-approved and well-characterised for sleep disorders
  • Long half-life gives all-day coverage from a single morning dose
  • Generally well tolerated at standard doses

Good to Know

A wakefulness agent, not a physique drug

Modafinil promotes alertness through atypical dopamine reuptake inhibition. It is FDA-approved (1998) for narcolepsy, obstructive sleep apnea and shift work sleep disorder, and popular off-label for focus. It has no direct anabolic, fat-loss or hormonal action - any training benefit is indirect, from being less tired.

Banned in-competition by WADA since 2004

WADA added modafinil to its Prohibited List on 3 August 2004; it is a prohibited stimulant (category S6) in competitive sport. A tested athlete using it for cognition/wakefulness would still fail an in-competition test.

Watch for rash - the one serious risk

Most side effects are mild (headache, nausea, anxiety, insomnia), but modafinil carries a rare risk of serious skin reactions including Stevens-Johnson syndrome. Any rash means stop and seek care. It is also Schedule IV and can be habit-forming.

Dosage Guidelines

Experience LevelDosage Range
Beginner100100 mg/day
Intermediate100200 mg/day
Advanced200400 mg/day
Frequency
Once daily in the morning (narcolepsy/OSA); ~1 hour before a shift (shift work sleep disorder)
Typical Cycle Length
112 weeks
Notes

Available as 100mg and 200mg oral tablets (Wikipedia); taken once a day with or without food (MedlinePlus). 200mg once daily is the standard approved dose; some sources cite up to 400mg but without clearly greater benefit. Take in the morning to avoid insomnia. May be habit-forming - do not exceed the prescribed dose or duration.

Side Effects

Headache

common
Severity
2/5

The most common side effect.

Mitigation

Hydration; lower dose if persistent.

Nausea

common
Severity
1/5

Mild gastrointestinal upset.

Mitigation

Take with food if needed.

Anxiety / Insomnia

common
Severity
2/5

Stimulant-related anxiety and trouble sleeping, especially with late dosing.

Mitigation

Dose in the morning; reduce dose.

Decreased Appetite

common
Severity
1/5

Reduced appetite is among the most frequently reported side effects in clinical trials.

Mitigation

Ensure adequate meal timing/intake even when appetite is blunted.

Serious Skin Reactions

rare
Severity
5/5

Rare but serious severe skin rashes, including Stevens-Johnson syndrome.

Mitigation

Stop immediately and seek urgent medical care at the first sign of rash.

General Mitigation Strategies

Take once in the morning to limit insomnia. Stop the drug and seek medical attention immediately if any rash develops (rare risk of Stevens-Johnson syndrome). It may be habit-forming, so use only at prescribed doses and durations.

Post Cycle Therapy (PCT)

PCT Not Required

Not a hormone and does not affect the HPTA. No PCT required.

How It Works

Modafinil is characterised as a central nervous system stimulant and an atypical dopamine reuptake inhibitor - it raises extracellular dopamine (and engages other wake-promoting pathways) to promote alertness. It is functionally a wakefulness agent rather than a classic amphetamine-type stimulant.

Fundamentals

WADA Status

Prohibited by WADA

In-Competition: Prohibited

Modafinil is banned in competitive sport by WADA, which added it to the Prohibited List on 3 August 2004 (per Wikipedia). It is a prohibited stimulant (S6), prohibited in-competition.
