What an aromatase inhibitor actually does
Testosterone (and other aromatizable androgens) are converted into estradiol by the enzyme aromatase. An AI like letrozole blocks that enzyme, so less estrogen is made body-wide. This is how you control estrogenic side effects (gyno, water retention, high blood pressure) on an aromatizing cycle. It does nothing for non-aromatizing compounds - there is no estrogen being produced from them to block.
Non-steroidal and reversible - unlike exemestane
Letrozole is a non-steroidal, competitive, reversible AI: it binds aromatase only while the drug is present, so when it clears the enzyme works again and estrogen can rebound. That is the opposite of the steroidal "suicidal" AI exemestane, which permanently destroys the enzyme. Reversibility matters when timing PCT - stopping letrozole abruptly can let estrogen bounce back.
The most potent AI - which makes it the easiest to crash estrogen
Letrozole is roughly 10-30x more potent than anastrozole and can suppress estradiol by >95%. That power is a liability: tiny dose changes swing estrogen dramatically, and it is very easy to crash E2 into a hole. It is best treated as an emergency/rescue AI (acute gyno, hyper-aromatizers, when anastrozole fails), not a first-line everyday choice.
Do not crash your estrogen - low E2 is worse than slightly high
Estrogen is essential for male joints, libido, erectile function, mood, lipids and bone. Crashed estrogen causes severe joint pain, fatigue, depression, anxiety, low libido and ED - and there is no quick fix except waiting for levels to recover. Many "high estrogen" symptoms people chase with an AI are actually low-estrogen symptoms from over-dosing one. Dose to bloodwork, not to feelings, and aim to control estrogen, not eliminate it.
AI vs SERM - different tools for different jobs
An AI lowers total estrogen everywhere (good for on-cycle estrogen control and emergency gyno). A SERM (tamoxifen, raloxifene, clomiphene) blocks the estrogen receptor in specific tissues and can restart the HPTA. For gyno, a SERM blocks the breast receptor without crashing systemic estrogen; for PCT you use a SERM, never an AI (crashing estrogen during recovery hurts it). For an acute gyno flare, letrozole plus a SERM is more effective than either alone.

These EOD "cycle assist" doses (0.25-1.25mg EOD) are off-label AAS-community consensus, not clinical-trial-derived - they are far below the FDA-labeled oncology dose (2.5mg/day, ~17.5mg/week), which is almost always too aggressive for bodybuilding use and is generally reserved for very short emergency bursts combined with a SERM. Steady-state plasma levels take 2-6 weeks to reach on daily dosing per the FDA label. Very easy to crash estrogen - start at the low end and titrate to bloodwork.
Side Effects
Crashed Estrogen

Due to extreme potency, easily suppresses estrogen to unhealthily low levels causing: severe joint pain, hot flashes, night sweats, fatigue, depression, anxiety, brain fog, panic attacks, loss of libido, erectile dysfunction, dry skin/eyes, paradoxical fat gain.
Use lowest effective dose. Get bloodwork. If crashed, stop letrozole and wait - no quick fix. May take weeks to recover.
Severe Joint Pain

Reduce dose. Ensure estrogen is not crashed. Joint supplements may provide minor relief.
Negative Lipid Effects

Always prefer anastrozole for routine estrogen management. Reserve letrozole for emergencies or when anastrozole fails. If you crash estrogen, there is no quick fix - you must wait for levels to recover.

Post Cycle Therapy (PCT)

How It Works
Letrozole competitively inhibits the CYP19A1 enzyme (aromatase) by binding to the cytochrome P-450 component, blocking the electron transfer chain and preventing conversion of androgens to estrogens. In a head-to-head comparative trial, letrozole (2.5mg/day) suppressed plasma estradiol by 95.2% vs 92.8% for anastrozole (1mg/day), and suppressed breast-tumor tissue estradiol by 97.6% vs 89.0% for anastrozole (Geisler et al., Clin Cancer Res 2008). Even with near-complete inhibition, some estradiol remains in men due to high testicular aromatase activity.
