Why this rating?
Follistatin binds and neutralizes myostatin, the protein that limits muscle growth. Myostatin-knockout animals show dramatic hypertrophy (roughly double muscle mass in mstn-null mice; Haidet et al., PNAS 2008), and by inhibiting myostatin, follistatin theoretically enables muscle growth beyond genetic limits, an effect impossible to achieve naturally. Follistatin (and other myostatin/activin-pathway inhibitors) is explicitly listed as prohibited by WADA. However, that striking evidence comes from AAV gene therapy and lifelong genetic knockouts sustaining follistatin expression for years, not from the short-half-life injectable peptide (FS-344) that research-chemical vendors sell. There is no controlled human trial showing injectable FS-344 builds muscle, so it is rated similarly to other mechanistically-promising but human-evidence-thin growth factor peptides (e.g. MGF) rather than near the steroids/IGF-1 tier.

Follistatin binds and neutralizes myostatin (and activin), the proteins that cap muscle growth. Myostatin-null animals (Belgian Blue cattle, 'mighty mouse' knockouts) carry roughly double the muscle mass. The theory is that inhibiting myostatin lets muscle grow beyond its normal genetic ceiling, an effect no natural training can reproduce.
The dramatic results are gene therapy, not the injectable peptide
The impressive, durable data (e.g. AAV1-FS344 gene therapy sustaining muscle gains for 2+ years) come from gene therapy and lifelong genetic knockouts, sustained follistatin expression. Injectable 'follistatin peptide' (FST-344/FST-315) is a different proposition: short half-life, unproven human bioavailability, and no good controlled evidence it reproduces those effects. It is also expensive and frequently counterfeited.
Do the gains persist after you stop?
Probably not, for the injectable. Myostatin suppression from a short-acting peptide is transient, so once dosing stops the brake comes back on and unsupported gains tend to regress, unlike the lasting myonuclei / 'muscle memory' a steroid cycle leaves behind. Even non-integrating gene therapy (e.g. episomal constructs) is described as temporary, with effects fading over roughly a year. There is no solid human follow-up showing permanent gains from the peptide.
Connective tissue is the weak link
If muscle force output climbs faster than tendons and ligaments adapt, injury risk rises. Progress loads gradually and support connective tissue (collagen + vitamin C) rather than chasing maximal weights early.
Anti-doping: named substance, not a gray area
Follistatin is explicitly named as a myostatin-binding protein under WADA's S4 (Hormone and Metabolic Modulators) category, in the "agents preventing activin receptor IIB activation" sub-section alongside other myostatin inhibitors. Anti-doping labs have published detection methods specifically for FS-344 (Reichel et al. 2019; Walpurgis et al. 2023). Follistatin gene therapy (AAV vectors) would separately fall under WADA's M3 Gene and Cell Doping category. It is firmly a banned, high-tier enhancement approach.

Detection Times
No established detection-window (days/weeks) data for injectable FS-344 in humans.
