Why this rating?

Enclomiphene stimulates natural testosterone production by blocking estrogen negative feedback at the hypothalamus, increasing LH/FSH release. It does not introduce exogenous hormones. It preserves/improves fertility unlike TRT. Used for PCT after cycles and as a TRT alternative. Rated 2 as it works entirely through natural pathways to restore endogenous testosterone production rather than replace it.

Overview

A SERM that boosts natural testosterone without suppressing fertility. The trans-isomer of clomiphene, it stimulates LH and FSH production more effectively with fewer side effects than Clomid.

Important Warnings

  • Not FDA-approved - the Androxal program was discontinued in 2021 and the EMA formally refused marketing authorization for EnCyzix in 2018, citing venous thromboembolism risk and insufficient evidence that it improves symptoms (bone strength, weight, libido, impotence) despite raising testosterone; compounded product quality/sourcing varies
  • Carries a rare but serious venous thromboembolism (DVT/PE) risk - avoid with a personal/family history of clotting disorders and seek immediate care for leg swelling/pain or chest pain/shortness of breath
  • Restarting the axis raises testosterone, which can raise estradiol - monitor E2 and add a low-dose AI only if bloodwork warrants
  • It is an axis-restart drug, not a dedicated gyno treatment - use tamoxifen/raloxifene for breast-tissue protection
  • Discontinue if any visual disturbances occur (rarer than with Clomid but possible)

How a SERM restarts your natural testosterone

The hypothalamus senses estrogen and slows GnRH when estrogen is high (negative feedback). A SERM like enclomiphene blocks the estrogen receptor at the hypothalamus, so the brain "thinks" estrogen is low and ramps GnRH back up. That drives the pituitary to release more LH and FSH, which tell the testes to make testosterone and sperm again. This is why SERMs are the backbone of PCT. They restart your own production rather than replacing it.

SERM vs AI: do not confuse them

A SERM (enclomiphene, tamoxifen, clomiphene) blocks the estrogen receptor at specific tissues and restarts the HPTA, that is what you want for PCT. An aromatase inhibitor (anastrozole, letrozole) lowers total estrogen everywhere by blocking its production, useful on-cycle to control estrogen, but the wrong tool for restarting the axis (and crashing estrogen during PCT hurts recovery). For PCT you reach for a SERM, not an AI.

Enclomiphene is the "clean half" of Clomid

Clomid (clomiphene) is a mix of two isomers: enclomiphene (the trans-isomer that does the useful antagonist work) and zuclomiphene (a long-half-life estrogenic isomer that accumulates over weeks and is blamed for much of Clomid's mood and vision side effects). Enclomiphene is the isolated pure trans-isomer, so it delivers the LH/FSH/testosterone boost with a cleaner side-effect profile and no lingering zuclomiphene build-up.

A fertility-preserving TRT alternative

Because it works by stimulating your own axis, enclomiphene raises testosterone while maintaining (or improving) LH, FSH and sperm production, unlike exogenous TRT, which shuts the testes down. That makes it attractive for younger or fertility-conscious hypogonadal men, though it is not FDA-approved (the Androxal program never gained approval).

Venous thromboembolism has been reported in trials: in a pooled safety population of 1,403 participants, deep vein thrombosis occurred in ~0.2%, pulmonary embolism in ~0.1%, and fatal ischemic stroke in ~0.1%. This risk was a central reason the EMA refused marketing authorization for enclomiphene (EnCyzix) in 2018.

Mitigation

Avoid if personal/family history of blood clots or clotting disorders. Seek emergency care for leg swelling/pain (DVT) or chest pain/shortness of breath (PE).

General Mitigation Strategies

Periodic vision checks recommended. Monitor estrogen levels. Watch for blood clot warning signs (leg swelling/pain, chest pain, shortness of breath) - this rare but serious risk was a key factor in the EMA declining to approve the drug. Generally well-tolerated with fewer side effects than Clomid otherwise.
