Overview
A first-generation SERM that is a mixture of two isomers: 62% enclomiphene (trans, antagonist) and 38% zuclomiphene (cis, agonist). Produces a stronger initial LH/FSH spike than tamoxifen but with more side effects, particularly vision issues.

How a SERM restarts natural testosterone
Clomiphene blocks the estrogen receptor at the hypothalamus, so the brain stops sensing estrogen's "slow down" signal. GnRH pulses ramp up, the pituitary releases more LH and FSH, and the testes resume making testosterone and sperm. Unlike TRT, it keeps intratesticular testosterone high enough to maintain fertility - which is why it doubles as both a PCT drug and a hypogonadism treatment.
The zuclomiphene problem - the reason enclomiphene exists
Clomid is not one molecule but two isomers: roughly 62% enclomiphene (the trans-isomer that does the useful antagonist work, ~10-hour half-life) and ~38% zuclomiphene (a cis-isomer with estrogenic activity and a very long half-life, around 30 days). Zuclomiphene accumulates with repeated dosing and is blamed for much of Clomid's mood swings, emotional sensitivity and vision issues. Pure enclomiphene simply removes that problematic isomer.
Clomid vs Nolvadex for PCT
Clomiphene produces a stronger initial LH/FSH spike than tamoxifen, but at the cost of more side effects (vision, mood). Tamoxifen is generally better tolerated and is superior for gyno. Many protocols run both together for ~30% faster recovery, combining Clomid's stronger axis kick with Nolvadex's cleaner profile and breast-tissue protection.
SERM vs AI - and why vision symptoms mean STOP
A SERM (clomiphene) restarts the axis by blocking estrogen receptors; an aromatase inhibitor lowers total estrogen - for PCT you want the SERM. Clomiphene's signature risk is visual disturbance (blurring, spots, scintillating scotomata), which is dose-dependent and usually reversible but has rarely been permanent. Any visual symptom is a reason to stop immediately, not to push through.

Side Effects
Vision Disturbances

One case: 33-year-old man on 100mg/day lost inferior visual field by day 14.
Ophthalmologic exam before starting. Discontinue immediately if any visual symptoms occur. Use lowest effective dose.
Mood Changes

General Mitigation Strategies
Vision issues are the primary concern - stop immediately if any visual symptoms occur. The zuclomiphene isomer (38% of Clomid) accumulates due to its long half-life and has estrogenic activity, contributing to side effects. Pure enclomiphene (not yet FDA-approved but available through compounding) eliminates the problematic isomer.
Post Cycle Therapy (PCT)
Clomiphene IS a PCT drug. Standard protocol: 50/50/25/25 (mg/day for 4 weeks). Can be combined with Nolvadex for enhanced recovery. Start 3-5 days after last oral steroid, or 2-3 weeks after last long-ester injection.
How It Works
Clomiphene competitively inhibits 17β-estradiol at estrogen receptors in the hypothalamic arcuate nucleus. This blocks negative feedback, increasing GnRH pulse frequency and amplitude, which stimulates the pituitary to release more LH and FSH. LH then stimulates Leydig cells to produce testosterone while FSH supports spermatogenesis. Unlike exogenous testosterone, clomiphene maintains intratesticular testosterone necessary for sperm production.

Detection Times
A controlled pharmacokinetic study (Miller et al. 2019) found the zuclomiphene metabolite detectable in urine for 121 to over 261 days after a 30-day, 50mg/day course - one of the longest urinary detection windows of any commonly used compound, with substantial variability between individuals. Blood detection window is not well characterized in the literature.
