Overview

A long-acting amylin analog developed by Novo Nordisk. Amylin is a satiety hormone released alongside insulin after meals, and it works through a different pathway than GLP-1, so cagrilintide complements GLP-1 agonists rather than duplicating them. It is best known as the amylin half of CagriSema, the fixed-dose once-weekly combination of cagrilintide 2.4mg and semaglutide 2.4mg, which produced 22.7% mean weight loss at 68 weeks in the REDEFINE 1 trial assuming full adherence (the treatment-policy estimate, which counts everyone regardless of adherence, was 20.4%).

Important Warnings

  • GLP-1/amylin class agents are contraindicated with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia type 2 (MEN 2)
  • Rapid weight loss causes meaningful lean-mass (muscle) loss unless protein intake is high and resistance training is maintained
  • Rebound weight gain is common after discontinuation without sustained lifestyle change
  • Stacking cagrilintide with GLP-1/GIP/glucagon agonists beyond the validated CagriSema combo multiplies GI, dehydration and muscle-loss risk

Amylin, not GLP-1

Cagrilintide is an amylin analog, a different satiety hormone from GLP-1. That is precisely why it is combined with semaglutide, two mechanisms, two receptors, additive appetite suppression rather than redundancy.

CagriSema is the real product

On its own cagrilintide is a moderate weight-loss agent (~9-11%). The clinically important entity is CagriSema (cagrilintide 2.4mg + semaglutide 2.4mg), which reached ~22.7% mean weight loss at 68 weeks in REDEFINE 1 and was filed with the FDA in December 2025.

Lean-mass loss during rapid weight loss

Like all satiety-driven agents, a substantial fraction of weight lost can be lean tissue if intake and training are not managed. High protein plus resistance training is what shifts the fat-to-lean loss ratio in your favor. This matters for anyone using it in a physique context.

No anabolic or hormonal action

It does not raise or suppress testosterone, does not aromatize, and has no effect on the HPTA. There is no AI or PCT consideration. It is purely a metabolic/appetite tool.

Rebound weight is the norm

Appetite returns after stopping. Without established dietary and training habits, weight regain is expected. The drug manages appetite, it does not permanently reset the setpoint.

In the Phase 2 monotherapy dose-finding trial, target doses of 0.3, 0.6, 1.2, 2.4 and 4.5mg once weekly were each reached via a dose-escalation period of up to 6 weeks to limit nausea. In the CagriSema combination it is fixed at 2.4mg alongside semaglutide 2.4mg, with both components titrated together over roughly 16 weeks (0.25 -> 0.5 -> 1.0 -> 1.7 -> 2.4mg, ~4-week steps) before reaching maintenance. Slow titration is the key to tolerability; compounded/research-chemical cagrilintide is commonly dosed and titrated on this same schedule.

General Mitigation Strategies

How It Works

Cagrilintide is an acylated (fatty-diacid) analog of human amylin that acts as an agonist at the amylin and calcitonin receptors. Native amylin is released with insulin from pancreatic beta cells and reduces food intake by slowing gastric emptying, suppressing glucagon, and promoting satiety via the area postrema in the brainstem. Because amylin signaling is distinct from and complementary to GLP-1 signaling, combining cagrilintide with semaglutide (CagriSema) engages two separate satiety systems for additive weight loss. The fatty-diacid acylation gives it a ~7-8 day half-life, enabling once-weekly dosing.

WADA Status Unclear

Cagrilintide is not explicitly named on the WADA Prohibited List. GLP-1 agonists (semaglutide, tirzepatide) sit on WADA's Monitoring Program as approved prescription drugs, but cagrilintide monotherapy has no approval from any regulatory health authority, which is exactly the condition WADA's S0 (Non-Approved Substances) category targets ("prohibited at all times... with no current approval by any governmental regulatory health authority for human therapeutic use"). That makes standalone cagrilintide a real S0 risk for tested athletes even though it is not named individually. Status is genuinely uncertain; verify before competition.
