Why this rating?

Per FDA prescribing information, cabergoline is a dopamine D2 agonist that controls prolactin levels, normalizing prolactin in 86-92% of patients. It is used to manage side effects from 19-nor steroids (trenbolone, nandrolone), not to enhance performance itself. FDA-approved for hyperprolactinemia. Does not build muscle or enhance athletic performance. Any testosterone/libido "restoration" it produces is only reversing prolactin-driven suppression caused by other compounds, not raising hormones beyond a natural baseline. Rated 1 - basically natty, organ-support medication for hormonal side effect management with no independent effect on the muscular/physique ceiling.

Overview

A potent dopamine D2 receptor agonist (ergot derivative) used to control elevated prolactin levels. Essential ancillary during cycles containing 19-nor steroids (trenbolone, nandrolone) which elevate prolactin. Known for its exceptionally long half-life allowing twice-weekly dosing and its superior tolerability compared to bromocriptine.

Important Warnings

  • Do not use with dopamine antagonists (metoclopramide, antipsychotics) - blocks effects
  • Cardiac valve risk is dose-dependent - keep doses at minimum effective
  • Baseline echocardiogram recommended for long-term use
  • Can unmask psychotic tendencies in predisposed individuals
  • Monitor for impulse control disorders (gambling, compulsive behaviors)
  • Do not abruptly discontinue after long-term use

Prolactin Control During 19-Nor Cycles

Essential for controlling prolactin elevation caused by trenbolone, nandrolone (deca), and other progestogenic compounds.

Sexual Function Restoration

Restores libido, erectile function, and gonadal function suppressed by hyperprolactinemia. Clinical studies (e.g., De Rosa et al.) document significant improvement in sexual potency and testosterone status within months of treatment.

Gynecomastia Prevention

Prevents prolactin-induced gynecomastia which can occur independently of estrogen-related gyno.

Refractory Period Reduction

Good to Know

It is for PROLACTIN, not estrogen

Cabergoline is a dopamine D2 agonist, dopamine tonically suppresses prolactin, so caber fixes prolactin-driven problems: 19-nor "deca/tren dick", prolactin-induced gyno and low libido. It does NOTHING for estrogen gyno, water retention or high E2, that is the job of an AI or SERM. Reaching for caber to fix an estrogen problem is a classic mistake.

Really only needed with 19-nors

Trenbolone and nandrolone are progestogenic and push prolactin up; caber is the classic pairing (start ~2 weeks before the 19-nor). Testosterone-only or DHT-derivative cycles rarely raise prolactin enough to need it, confirm with bloodwork rather than running it by default.

Caber vs prami: the trade-off

Cabergoline is ergot-derived, extremely long-acting (twice-weekly dosing) and more potent/convenient for prolactin, but carries a dose-dependent 5-HT2B cardiac-valve fibrosis risk. That risk is a genuine concern only at the multi-mg/DAY Parkinson's doses, bodybuilding doses (0.25-0.5mg twice weekly) are far below that. Pramipexole is the non-ergot alternative (no valve risk) but is daily, less potent, and has higher impulse-control-disorder risk.

Do not crash prolactin either

Some prolactin is needed for normal libido and mood; over-suppressing it causes its own low-libido and sexual dysfunction. Dose to labs, aim for mid-normal prolactin, and get bloodwork to confirm you actually need it.

FDA-approved titration for hyperprolactinemia: start at 0.25mg twice weekly (0.5mg/week total), increase by 0.25mg twice weekly at intervals of at least 4 weeks per prolactin bloodwork, up to the FDA-approved maximum of 1mg twice weekly (2mg/week total). In PED use, begin ~2 weeks before starting 19-nor compounds (trenbolone, nandrolone) and continue throughout the cycle; going beyond the 2mg/week ceiling is off-label and not typical practice. Tablets are typically scored 0.5mg - split accordingly.

Activation of 5-HT2B receptors can cause valve fibrosis, most strongly linked to HIGH doses (>2mg/day, as used off-label in Parkinsons - cabergoline is not FDA-approved for Parkinsons). FDA labeling notes rare cases have also been reported at lower hyperprolactinemia doses (0.5-2mg/week), so risk is not zero at bodybuilding doses, but is far lower than at Parkinsons-level daily dosing.

Mitigation

Keep doses at minimum effective. Baseline echocardiogram recommended before starting and for long-term use. Risk is dose-dependent - bodybuilding weekly totals are far below the daily Parkinsons doses linked to most reported cases.

General Mitigation Strategies

At bodybuilding doses (0.5-2mg/week), cabergoline is generally well-tolerated. The cardiac valve risk that caused concern in Parkinsons patients (doses >2mg DAILY) is much lower at typical PED support doses, though FDA labeling notes rare cases even at hyperprolactinemia doses. Take with food to reduce nausea. Start at minimum dose and titrate based on bloodwork. Only ~4% (3.9%) discontinuation rate due to adverse effects in the Verhelst 1999 455-patient study.

Post Cycle Therapy (PCT)

PCT Not Required

Cabergoline is an ancillary compound used during cycle, not a PCT drug. It does not affect the HPT axis directly. Continue use during PCT if prolactin is still elevated. Can be discontinued once prolactin normalizes.

How It Works

Cabergoline mimics dopamine by binding to D2 receptors on lactotroph cells in the anterior pituitary, directly inhibiting prolactin secretion. Activation of D2 receptors leads to inhibition of adenylyl cyclase activity and decreased intracellular calcium. Also has affinity for D3, D4, and various serotonin receptors (5-HT1A, 5-HT2A, 5-HT2B, 5-HT2C). Prolactin suppression begins within 3 hours of dosing, with effects lasting up to 7 days.
