This is berberine's main claim to fame in a physique context. MK-677 and growth hormone reliably raise fasting blood glucose and blunt insulin sensitivity. Berberine, via AMPK activation, is the standard over-the-counter tool to keep blood sugar in range on those compounds (metformin is the pharmaceutical alternative). If you run MK-677, berberine + diet is the usual glucose-management answer.
The CYP3A4 interaction is the real safety issue
Berberine inhibits the CYP3A4 enzyme and P-glycoprotein. The machinery that clears a large fraction of prescription drugs. That means it can raise blood levels of co-administered medications (cyclosporine, some statins, certain blood thinners, and more) toward toxic ranges. This is a more important caution than its GI effects. Always check drug interactions before combining berberine with any prescription.
"Nature's metformin" is earned, not just marketing
The nickname reflects genuine head-to-head data: Yin et al. (2008) found berberine 500mg 3x/day matched metformin for HbA1c and fasting-glucose reduction over 13 weeks, and meta-analyses support comparable glycemic efficacy. Berberine also lowers LDL/triglycerides, which metformin largely does not.
Poor bioavailability drives the dosing and the GI effects
Less than ~1% of oral berberine is absorbed, so it must be dosed 2-3x/day and much of it stays in the gut, which is exactly why GI upset is the most common side effect. Dihydroberberine is a more bioavailable alternative that some use at lower total doses to reduce GI issues.

Standard clinical dose is 500mg 2-3x/day (1,000-1,500mg total) taken WITH meals. Splitting doses is necessary because of the short action and poor bioavailability, and taking it with carb-containing meals targets the post-meal glucose spike. Fasting glucose improves within 1-2 weeks; full HbA1c effect takes ~3 months. Dihydroberberine is a more bioavailable form some use at lower doses. Many cycle it (e.g. 8-12 weeks) rather than run it indefinitely, though continuous use is common in diabetes management.
Plasma half-life is ~4-8 hours (biphasic elimination); combined with very poor oral bioavailability (<1%, extensive first-pass metabolism), levels need topping up 2-3x/day for continuous effect.
Side Effects
GI Upset

Hypoglycemia (in combination)

General Mitigation Strategies
GI effects are the main nuisance, mitigated by taking berberine with food, splitting into 2-3 daily doses, and titrating up slowly. The more serious concern is pharmacological: as a CYP3A4/P-glycoprotein inhibitor it can dangerously raise levels of other medications, and it can compound the glucose-lowering of antidiabetic drugs. Anyone on prescription medication should clear berberine with a professional first.

How It Works
Berberine activates AMP-activated protein kinase (AMPK), the cellular energy sensor also targeted by metformin and exercise. AMPK activation improves insulin sensitivity, increases glucose uptake, and suppresses hepatic gluconeogenesis and lipogenesis (lowering glucose, LDL, and triglycerides). It also modulates the gut microbiome and inhibits DPP-4. Note: some glucose-lowering appears partly AMPK-independent, so "AMPK activator" is the headline mechanism but not the whole story. Oral bioavailability is very low (<1%), which shapes both dosing and its side-effect profile.
