Why this rating?

Anastrozole blocks the aromatase enzyme, reducing estrogen conversion from testosterone. It does not add exogenous hormones. It modulates natural hormone balance. Per FDA labeling, the 1mg/day clinical dose reduces estradiol by ~70% within 24 hours and ~80% after two weeks, with whole-body aromatase inhibition around 97% in cross-over studies; off-label AAS/TRT-adjunct doses are typically much lower (often 0.25-1mg every other day). Used as a supportive compound during TRT or steroid cycles for estrogen management, not direct performance enhancement. Rated 1 (basically natty) because it does not build muscle, burn fat, or raise anabolic hormones on its own - it only manages estrogen side effects from other compounds and has no independent effect on the natural physique/performance ceiling.

Overview

A third-generation non-steroidal aromatase inhibitor (AI) that competitively and reversibly inhibits the aromatase enzyme. The most commonly used AI for estrogen control during steroid cycles due to its balance of efficacy and manageability. At 1mg/day, reduces estradiol by ~80% (per FDA labeling), with aromatase inhibition around 97% in clinical studies.

Important Warnings

  • Reversible binding means estrogen can rebound when stopped
  • Crashing estrogen is worse than slightly elevated estrogen for most men
  • Many men do not need an AI even on TRT - get bloodwork before using

Estrogen Control During Cycle

Prevents estrogen-related side effects (gynecomastia, water retention, high blood pressure) during aromatizing steroid cycles.

Gynecomastia Prevention

Reduces circulating estrogen to prevent breast tissue development. Note: Less effective than SERMs for treating existing gyno.

Water Retention Reduction

By lowering estrogen, reduces subcutaneous water retention for a drier, more defined appearance.

TRT Adjunct

Used in testosterone replacement therapy when estradiol levels become elevated, though this is now considered less necessary than previously thought.

What an aromatase inhibitor actually does

Aromatase is the enzyme that converts testosterone into estrogen. An AI blocks that enzyme, lowering total estrogen systemically, used on aromatizing cycles to prevent gyno, water retention and high blood pressure. It only helps with compounds that actually aromatize.

Do NOT crash your estrogen, the classic mistake

Estrogen is essential for libido, mood, joints, bone and lipids. Over-suppressing it ("crashing E2") causes flat mood, no libido, achy joints and worse cholesterol, often mistaken for high estrogen and "fixed" with more AI, making it worse. Dose to bloodwork toward a target range, not to zero.

AI vs SERM, and many people do not need one

An AI lowers estrogen production everywhere; a SERM (Nolvadex) instead blocks estrogen only at tissues like breast, better for gyno without crashing systemic E2. On sensible testosterone doses many people need no AI at all; confirm with labs before using one.

Start low (e.g. 0.25mg EOD or 0.5mg twice weekly) and titrate based on estradiol blood tests, not symptoms alone. Target E2 of roughly 20-30 pg/mL for most users. Most men on properly dosed TRT do not need an AI at all - one study of men on testosterone therapy found only ~2.6% had estradiol elevated enough (>60 pg/mL) to warrant anastrozole, dosed there at 0.5mg three times weekly. This off-label range is far below the FDA-labeled oncology dose (1mg/day, ~7mg/week), which is not the reference point for AAS/TRT ancillary use. Estradiol changes begin within 3-4 days of a dose adjustment; a new steady state takes about 7-10 days.

General Mitigation Strategies

The key is not to over-suppress estrogen. Start with a low dose (0.25-0.5mg twice weekly) and adjust based on bloodwork, not just symptoms. Target estradiol of 20-30 pg/mL. Testosterone to estradiol ratio of 10:1 or greater is optimal. Many symptoms attributed to "high estrogen" are actually low estrogen from AI overuse. Keep Nolvadex on hand for acute gyno symptoms rather than increasing AI dose.

Support Supplements

Ancillary supplements commonly run alongside Anastrozole to manage side effects, support the target tissue, or fill nutrient demands it creates.

Omega-3 fish oil

Meta-analysis shows anastrozole modestly lowers HDL with extended use (no significant LDL change), consistent with estrogen's role in lipid metabolism. Omega-3s support triglycerides and general cardiovascular health as a hedge against that.

Post Cycle Therapy (PCT)

PCT Not Required

Anastrozole is typically discontinued during PCT as SERMs (Nolvadex, Clomid) are used to block estrogen at receptors while stimulating natural testosterone production. Some protocols include low-dose AI early in PCT.

How It Works

Anastrozole binds reversibly to the cytochrome P-450 component of the aromatase enzyme (CYP19A1), blocking the conversion of androgens (testosterone, androstenedione) to estrogens (estradiol, estrone). Unlike steroidal AIs, it does not destroy the enzyme - inhibition is reversed when the drug clears. Has no intrinsic estrogenic, androgenic, or progestagenic effects. Highly selective for aromatase with negligible effects on other CYP450 enzymes.
